Objective: to report a clinical case of a patient with advanced extra-articular tophaceous gout, highlighting the complexity of diagnosis and management of this condition. Method: this is a descriptive study, with a narrative and reflective approach, based on the patient’s clinical presentation and progression. Results: the patient, with chronic gout and multiple comorbidities, presented with extensive deposition of monosodium urate crystals in uncommon extra-articular sites, including skin, cervical spine, and cricoarytenoid cartilage. The disease evolved with local complications, risk of infection, functional impairment, and significant impact on quality of life, in addition to overlapping cardiovascular and metabolic factors that worsened the prognosis. Diagnosis was supported by imaging tests, which were essential to characterize the deposits and monitor disease progression. Despite therapeutic measures, the condition remained refractory, with unfavorable outcomes. Conclusions: extra-articular tophaceous gout represents a rare manifestation, yet associated with high morbidity and mortality, requiring early diagnosis, multidisciplinary follow-up, and individualized therapy. This case highlights the importance of careful investigation of atypical manifestations, strict control of the disease and its comorbidities, as well as preparation of both healthcare teams and family members when facing a chronic, severe, and potentially disabling conditionReferences
