Objectives: To analyze the clinical and epidemiological profile, instituted treatments, and clinical outcomes of patients hospitalized with decompensated HF in a public hospital in the Federal District, emphasizing factors associated with disease severity and length of stay. Methods: This was an observational, cross-sectional, and analytical study conducted between January 2024 and January 2025, including 115 patients admitted to the ward, red room, or emergency unit of the Regional Hospital of Sobradinho (HRS-DF). Adults aged ≥18 years with a principal diagnosis of decompensated HF (ICD-10 I50/I50.9) were included. Exclusion criteria were stable secondary HF, hospital stays <24 hours, heart transplantation, or exclusive palliative care. Data were obtained from electronic medical records and analyzed using descriptive and inferential statistics (chi-square and Fisher tests) with Jamovi, RStudio, and Excel software. Results: Most patients were male (53.9%), with a mean age of 69.4 years and left ventricular ejection fraction <40% in 49.6%. The most prevalent comorbidities were hypertension (81.7%), diabetes (35.7%), and anemia (34.8%). In-hospital mortality was 16.5%. Therapeutic adherence was associated with better outcomes (p=0.038), while valvular etiology correlated with higher mortality (p=0.035). Advanced support use was strongly associated with death (p<0.001). Conclusion: The clinical profile of HF patients within the SUS reveals major care gaps, highlighting low adherence, underuse of evidence-based therapies, and delayed diagnosis. Integrated management strategies are essential to improve outcomes and reduce the burden of HF in the public health system.
