Relationship between loss of lean mass, muscle strength and functional capacity in critical trauma patients

Abstract

Objective: To analyze the relationship between lean mass loss occurring in the ICU and the strength and functionality of critically ill trauma patients. Methodology: This is a descriptive, observational and prospective cohort study carried out in a Neurotrauma Intensive Care Unit (ICU), with trauma patients aged over 18 years. The rectus femoris thickness was evaluated by ultrasonography images on the first (D0) and seventh (D7) day of admission to the ICU, these data were compared with association measures with scores from the Medical Research Council Scale (MRC) and ICU Mobility Scale (IMS). Results: The study included 34 patients, mostly male (88.2%) with a median age of 43.7, there was an 9% reduction in muscle thickness from D0 to D7. Patients who had greater loss of lean mass had lower MRC on awakening and those who died had significantly lower BMI both on awakening and discharge than those who survived. Conclusion: There may be a directly proportional relationship between ICU lean mass depletion and strength and functional capacity, demonstrating that muscle weakness and loss of functionality can be predictors of mortality.
